Passport Services

The Assessor's Office now provides passport services to our community. The Department of State has designated the Town of Ashland as an official passport application acceptance agent.


Make an Appointment

We will accept applications for processing and to administer oaths for passport purposes by appointment only. Please call 508-881-0100, ext. 2012 to make an appointment.

What You Will Need

In general, you will need the following items to apply for a passport:
  • Application for Passport, Form DS-11 (do not sign in advance)
  • Proof of U. S Citizenship (Certified copy of Birth Certificate)
  • Proof of Identity (valid Driver's License or old passport)
  • 1 Passport Photograph 
  • Social Security Number
  • Applicable Fee

Processing Time

Please note that the current processing time is approximately 8- 11 weeks. However, an expedited passport can be processed in 5- 7 weeks for an additional $60. To process passports at the Ashland Assessor's Office, 2 checks or money orders are required.

Fees

  • For age 16 and over, make 1 check payable to the "U.S. Department of State" for $130 and 1 check payable to the "Town of Ashland" for $35. If you wish to expedite the passport, add an additional $60 to the $130 for a total of $190.
  • For age 15 and under, make one check payable to the "U.S. Department of State" for $100 and 1 check payable to the "Town of Ashland" for $35. If you wish to expedite the passport, add an additional $60 to the $100 for a total of $160.

Minors

Minors (age 15 and under) are required to have both parents in attendance. If 1 parent can not attend, a notarized Consent Form must be filled out for each child and a copy of the parent's driver's license, front and back, on the same sheet.
